Start with a short summary.

Enter your first and last name, email address, state, topic, and a brief summary, then review the required consent checkbox. These required items are marked * or explained beside the checkbox. Your phone number and the company/other-party field are optional. Choose “I’m not sure yet” if you cannot identify the topic; you can still explain what happened.

Use an email address you can access and check it for typing errors. The summary accepts 20–1,800 characters, including spaces; the counter shows how much you have entered. This form has no attachment upload. Leave out sensitive identifiers, passwords, and confidential documents. The guide below explains which facts are useful.

What should I include? What happens after I send?

A useful first message

A few clear facts
are enough to begin.

Use this first message to explain the problem and the next step you want help considering. The summary field allows 20–1,800 characters, including spaces. Focus on the main issue, any approaching date, and the types of records available. You can say when a detail is unknown.

  1. Your details and location

Enter your first and last name, an email address you can check, and your state. A phone number is optional; include one if you want the office to have a way to call you. If the transaction, property, or court proceeding is in a different state, identify that location in your summary so the firm can assess the inquiry.

  2. The other party and topic

Name the lender, seller, contractor, collector, or other party involved if you know it. If several companies are involved, briefly explain each one’s role—for example, who sold the service and who collects the payments. If you are inquiring for a business, include its name and your role. Choose the closest topic, or select “I’m not sure yet.” You do not need to identify a legal claim.

  3. What happened and when

Explain what was expected, what actually happened, and when you first noticed the problem. Mention the main steps you already took, such as a written dispute, refund request, or complaint, and the response you received. If a date is approximate, label it that way. Say whether another lawyer, a court case, or an agency complaint is already involved so the firm can understand the current situation.

  4. What you want to resolve

Describe the practical result you hope to discuss: a corrected record, an explanation of a charge, a refund, completed work, or another clear goal. Include the amount involved if known and distinguish a confirmed amount from an estimate. If a notice lists a hearing, response, sale, or other important date, identify that date and the type of notice. Asking for help does not change the date or guarantee a particular result.

Use this simple outline in your own words

“I live in [state]. In [month], I bought [product or service] from [company]. I expected [main promise], but [what happened]. I contacted the company on [date] and received [response, or no response yet]. I have [types of records]. I would like help understanding [goal]. A [type of notice] lists [date], if relevant.”

Replace the brackets with facts that fit your situation. “I do not know yet” is better than guessing. If space is limited, keep the main problem, your goal, and any approaching date. Save a copy for your own records; leave out passwords, complete account numbers, and confidential documents.

After you reach out

Know what the receipt means
and what comes next.

  1. Check for a receipt

After a successful online submission, the form is replaced by a thank-you message and a reference number. Save that number so the office can identify the inquiry when you follow up. It confirms that the online service received the inquiry for processing, not that an attorney has read it, a consultation is scheduled, or an email has reached your inbox.

  2. Understand the initial review

A separate email confirmation with the firm’s contact card should follow. Initial review considers the issue, its location, whether it fits the firm’s services, and any conflict that could prevent assistance. A conflict can arise from the firm’s relationship with another person or business involved. The team may need more information before deciding whether further review is appropriate. The email itself is not a case assessment.

  3. Discuss any proposed representation

If further review is appropriate, the firm may ask questions or request records before discussing possible next steps. If representation is offered, ask exactly what work is included, what is outside the agreed scope, how fees and other costs are handled, and how communication will work. Representation requires a separate written agreement. Until that agreement is in place, do not assume the firm is contacting another party, monitoring dates, or making a filing for you.

Keep the firm’s details handy

The “Save contact” links download a vCard file containing Lex Bellator’s name, phone number, email address, and website. Open the downloaded file in your contacts application, check the information, and choose whether to save it. Your device may ask you to confirm the import. Saving the card does not send information to the firm, start an inquiry, or book a consultation.

For a question, correction, or update, call (888) 742-2880 or email info@lexbellatorgroup.com. Give your name, the email address used for the inquiry, and any reference number. Identify what has changed rather than submitting the same inquiry repeatedly. If a date is approaching, explain the date and type of event when you call; do not rely on an automatic acknowledgment as a response to an urgent issue.

Questions before
you contact us.

Is the initial case evaluation free?

You can request an initial case evaluation without an evaluation fee. That does not mean the firm has agreed to take the matter. Any later legal work, fee arrangement, and other costs must be discussed and agreed to separately in writing. Before proceeding, ask what work is included, how fees are calculated, when payment would be due, and which expenses you may be responsible for.

Can I inquire if I live outside California?

Yes. Provide your state and explain where the transaction, property, or proceeding is located if it is somewhere else. Whether the firm can assist depends on the matter, the jurisdiction—the location or legal authority involved—attorney authorization, conflicts, and case acceptance. The state choices do not promise representation in every location. If you are unsure which location matters, explain what you know.

Can I attach records, or do I need every document first?

This form accepts a written overview, not attachments. You do not need every document to make an inquiry. Identify useful records, such as an agreement, notice, statement, or written response, and explain what is missing. Keep originals and complete copies, including dates and all pages. If records are requested, ask how to provide them. Do not send sensitive or confidential documents through the initial form or ordinary email.

What if I received a reference number but no confirmation email?

Keep the reference and check the inbox, spam, and junk folders for the address you entered. The email is processed after the online receipt, so a reference does not prove delivery. If the confirmation does not arrive or your details need correcting, call or email with your name, reference, and the detail to update. Do not send the entire inquiry again or include sensitive records. A receipt is not a booked appointment or case acceptance.

What if the page shows an error or I am unsure the form was received?

Read the message next to the form. Correct missing fields or an invalid email address if asked. A connection error or timeout means receipt was not confirmed; it does not always mean that nothing arrived. Follow the instructions to retry without changing the information, or call to check. If you already have a reference number, use it for follow-up instead of sending a duplicate inquiry.

Why does the button say “Continue by email”?

When direct online submission is unavailable, this option prepares an email for you to review and send in your own email application. Opening the draft does not send it. This route does not generate the form’s reference number or automatic confirmation. Check the recipient and your details before sending. If no application opens, email info@lexbellatorgroup.com directly or call (888) 742-2880.

When should I expect a personal response?

An automatic acknowledgment is separate from a response by a person. Review time can vary; this page does not promise a fixed response time. Call or email with your name and reference to check the status. You may mention a preferred callback time, but that does not reserve an appointment.

What if a deadline is approaching before I hear back?

Call and identify the date and type of notice or event. A message or acknowledgment does not extend a deadline or confirm that the firm is handling it. Keep track of the date and seek timely help rather than waiting for review. If a lawyer already handles the matter, raise the deadline with that lawyer.

What if I need help using the form or another way to contact the office?

Call (888) 742-2880 or email info@lexbellatorgroup.com if the page is difficult to use. Describe the step that caused the problem and your preferred way to communicate so the office can discuss available options. You do not need to provide a diagnosis or sensitive records to explain a website problem. The accessibility page has additional information about reporting a barrier.

For more detail, read the privacy notice about information provided through the website, the general questions about working with the firm, or how the firm approaches a review. Ready to explain your situation? Return to the inquiry form.

One more step

Your request is ready to send.

Direct online delivery is unavailable here. Choose “Continue to email” to open a draft addressed to info@lexbellatorgroup.com in your email application. Review its contents and press Send in that application. Nothing has been submitted by this page, and this email-draft option does not produce the form’s automatic receipt.

If an email application does not open, start a new message to info@lexbellatorgroup.com and include your name, contact details, and brief summary. You can also call the office. Opening or closing this window does not send anything.
